BY PROVIDING YOUR MOBILE PHONE NUMBER TO UNIVERSAL MEDICAL GROUP AND OPTING IN, YOU CONSENT TO RECEIVE TEXT MESSAGES (SMS/MMS) AS DESCRIBED BELOW. CONSENT IS NOT A CONDITION OF PURCHASE OR OF RECEIVING CARE.

Program Description

Universal Medical Group ("we," "us," or "our") sends text messages to patients and prospective patients who have opted in. Messages may include:

How to Opt In

You may opt in by providing your mobile phone number to us through any of the following: an intake or registration form, a request for an appointment, a verbal authorization to a staff member, a reply of YES, START, or JOIN to one of our messages, or by enrollment through our patient portal.

Message Frequency

Message frequency varies and depends on your interactions with our practice (for example, scheduled visits, refill requests, or billing activity). You should not expect a fixed number of messages per period.

Message and Data Rates

Message and data rates may apply. Charges are billed by and payable to your mobile service provider. Universal Medical Group is not responsible for delays or failures in delivery of any messages, as delivery is subject to effective transmission by your wireless carrier. We are not liable for charges from your carrier.

Supported Carriers

The program is generally available on major U.S. carriers, including AT&T, T-Mobile, Verizon Wireless, Sprint, Boost, U.S. Cellular, MetroPCS, Cricket, and others. Carriers are not liable for delayed or undelivered messages.

Privacy and Confidentiality

Mobile phone numbers and message content are treated in accordance with our Notice of Privacy Practices. We do not share or sell mobile phone numbers or SMS opt-in data to third parties or affiliates for marketing purposes. Messages are sent over standard cellular networks and are not encrypted; please consider this when responding with personal or health information. Avoid sending clinical questions, symptoms, or emergency information by text.

Emergencies

Do not use text messaging for medical emergencies or urgent clinical issues. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. For urgent but non-emergency clinical questions, please call our office.

Changes to This Policy

We may update this SMS Consent & Terms page from time to time. The revised version will be posted on this page with an updated Effective Date.
